Data Protection Policy
- Lorin Lakasing Ltd is registered with the ICO – reference no: ZA366073
- Personal data of patients is kept in digital form and hard copy. Data necessary for patient care including name, date of birth, address, contact details, details of other medical practitioners involved in care, as well sensitive data including medical history and notes / correspondence and laboratory and scan results. For insured patients details of their medical insurance company are also kept.
- Patient data is kept securely. Medical notes are stored in a locked office when not in use and digital notes are on a patient management database which is hosted by Imperial College Healthcare which is password protected. It can only be accessed by Lorin Lakasing and named secretarial / administrative staff working within the practice. Practice staff are trained in information governance and confidentiality.
- No data is used for marketing purposes unless specific written consent has been obtained.
- Patients consent is requested to use the patient’s email and phone number for the purposes of patient care only.
- Data is not shared with other organisations other than in the following circumstances:
- correspondence with relevant medical practitioners and other specialists directly involved in provision of medical care.
- laboratories or ultrasound scan clinics where specific tests and investigations necessary for the provision of care are performed. Information exchanged will be limited to only that which is necessary to assist interpretation of results.
- explicit consent has been given by the patient to share information requested by insurance companies to process a claim.
- for the purposes of unpaid invoices only, the patients name, address and contact details may be passed on to a debt collection agency.
- The privacy policy is sent to all patients after their Booking appointment.
- Patient notes are kept for 25 years and in line with GDPR, data subject rights include the request for medical notes to be deleted or corrected if inaccurate.
